After your home care assessment

We understand the aged care system can be difficult and time consuming to navigate. If you’ve recently received approval for a Home Care Package, our home experts can help.

Once you have been assessed as eligible for a Home Care Package, you will receive a confirmation letter from My Aged Care and placed on the national waitlist. Your position on the waitlist is based on your needs and circumstances, as well as the time you have spent waiting for home care.

If you would like to receive care while you wait, please enquire about our private services which can be purchased to provide you with the support you need until you receive your Home Care Package.

Once you reach the top of the waitlist, My Aged Care will send you a letter with your unique referral number and Home Care Package details. From here, you have 56 days to select Baptistcare as your preferred home care provider by calling our friendly Customer Centre on 1300 660 640 and quoting your referral number.

If you haven’t already met with us, one of our home care experts will arrange a time to visit you to discuss your care plan and how we can support you to stay active, independent and living in your own home.

We work with you to design a care plan that’s just right for you. During our initial visit we will:

  • get to know you and understand your goals and care needs
  • listen to your preferences about how you wish to receive care and offer guidance and support to help you choose the services that best meet your needs
  • take the time to compare all your options to get the most from your government subsidy and budget
  • answer any questions you may have about home care services and help guide you through the next steps.

You are welcome to have another person, such as a carer, family member or advocate, with you to help design your care plan.

Once you are happy with the design of your care plan, you will need to sign a Home Care Agreement for your services to begin. This agreement documents the details of what your Home Care Package will provide, who will provide the services and how much the services will cost.

After signing your Home Care Agreement you will be provided with a personalised care plan before you services commence.

We understand that your care needs may change over time and will review your care plan on an ongoing basis to make sure it’s still aligned with your needs. Your care plan cannot be changed without your agreement.

You can ask for a review of your care plan at any time if your needs change.
