Individual Recovery Planning
Consumers of Baptistcare’s Mental Health Services continue their recovery journey with the creation of their own personalised Individual Recovery Plan (IRP). The IRP puts structure to a consumer’s recovery goals by identifying and mobilising their:
- Skills and strengths
- Recovery goals
- Crisis plan
- Carers and supporters
- Community supports
- CALD supports and networks
When implemented within the context of a supportive mentoring relationship, an IRP can help a person manage their illness better and to remain living in and connected with their carers and community.
Community Mentor Program
Community Mentors engage with referred consumers and their carers in order to develop and implement Individualised Recovery Plans.
The IRP is the means by which consumers and carers negotiate the type of support they need to stay well, prevent/manage a crisis, maintain an independent lifestyle and stay connected with their community and supporters.
Supportive Landlord Service
Within the context of an Individualised Recovery Plan, Baptistcare acts as a ‘supportive landlord’ for people with a mental illness, by providing supportive property management and tenancy services
Currently Baptistcare manages a portfolio of 24 properties in the Central West region of WA.
Compass House
Based in Geraldton, Compass House is staffed 24 hours a day and provides short term crisis accommodation for referred consumers with a diagnosed severe and persistent mental illness.
