How Do I Access These Services?
Anyone can request a community aged care package, however prospective clients must first be assessed by an Aged Care Assessment Team (ACAT) to establish the appropriate level of care.
For more information about obtaining an ACAT referral:
- Contact your GP
- Call Commonwealth Carelink on 1800 053 222
- Visit the Commonwealth Respite and Carelink Centres website
- Locate your nearest ACAT, using the Commonwealth Government’s Aged Care Australia website
If a CACP is appropriate for your needs, the Aged Care Assessment Team will refer you to a service coordinator like Baptistcare.
