Baptistcare’s Hand in Hand provides help and support to people, their carers and immediate family in their own home to complete everyday tasks such as cleaning, meal preparation and shopping. This assistance enables them to remain in their own choice of residence and can delay or prevent the need to move to a hostel or nursing home.

Examples of the assistance that Hand in Hand can provide include:

Personal Care - Assist with hygiene needs, medication prompts
Home Help - Cleaning, laundry, shopping
Meal Preparation
Transport - Appointments, social outings, shopping
Respite - In home, for the main carer or client

Our highly professional Care Coordinators and Support Workers are fully trained and qualified to meet the care needs of the aged in their own homes and receive full supervision and support to ensure that the very best possible standards of service are delivered at all times. All Baptistcare staff are fully covered by insurance. Enormous emphasis is placed upon preserving the privacy and dignity of all care recipients and all care arrangements are kept in the strictest confidence.

There are several avenues through which you may receive services from Hand in Hand:

Community Aged Care Packages (CACP)

Subsidised by the Commonwealth Government, a CACP is an individually tailored package of care services which helps people with lots of different care needs to stay living at home by co-ordinating the different services they need. Anyone can request a community care package, however prospective clients must first be assessed by an Aged Care Assessment Team. To be referred to an ACAT Team contact your GP or call Commonwealth Carelink on 1800 053 222. If a CACP is appropriate for your needs, you will be referred to a service coordinator like Baptistcare. In most cases, you will be charged a small fee for your CACP.

Veterans' Home Care (VHC)

Supportive services provided to Veterans and eligible significant others through a Department of Veterans' Affairs referral. Again, in most cases you will be charged a fee at a subsidised rate.

Private Services

Baptistcare offers services by private arrangement with those who wish to apply. We can tailor an individual care plan to meet your specific needs that will enable you or a loved one to remain at home for as long as possible. In this instance an ACAT assessment is not required. Fees for private services are non-negotiable.
